Financial Assistance
Jacobson Memorial Hospital Care Center has adopted a charity care/financial assistance program to provide discounted healthcare services to individuals who meet established eligibility guidelines.
To enroll in an automatic payment plan and submit charity care application, contact the billing office at 701-584-7235.
Medical Records
To receive copies of your medical records or to have your records transferred to another provider, complete a release of information form.
