Jacobson Memorial Care Center, through the operation of Jacobson Memorial Hospital, is a leading provider of short-term post-acute medical care and rehabilitation and long-term skilled nursing care. We offer a high quality medical care through our Family Practice and Internal Medical Doctors along with our registered (RN) and licensed practical (LPN) nurses and certified nursing assistants (CNAs) in concert with physical and occupational rehabilitation therapists.

Philosophy
The Jacobson Memorial Care Center is a resident-focused long-term care facility. It is the home of 25 senior residents. Our caring staff celebrates the aging process, utilizing a wholistic approach to integrate body, mind, emotion and spirit. We are dedicated to continually finding new ways to enhance our residents' experiences while living in our Home.
