Full Time With Benefits:
Benefits are available to those employees who are regularly scheduled to work at least 32 hours per week. Medical insurance is provided through Blue Cross Blue Shield.
Part Time With Benefits:
Benefits are available to employees who are regularly scheduled to work at least 20 hours per week. Medical insurance is provided through Blue Cross Blue Shield.
Used for sick-calls, vacations, and high acuity/census days. No benefits are available to these positions.
Used to replace employees gone for short periods of time for vacations and medical or maternity leaves. No benefits are available to these positions.
Benefit Plans Policy
Employees who work at least 20 or more hours per pay-week are elegible to participate in any of the following benefit plans offered by Jacobson Memorial Hospital Care Center.
Sanford Health Insurance
- Signing up within 30 days from date of hire.
- The anniversary date which is March 1st of each year.
- If your previous insurance company dropped you at no fault of your own.
Premiums may be deducted from your paycheck on a pre-tax basis. Jacobson Memorial Hospital Care Center pays for a percentage of the premium depending on your work status of part-time or full-time and depending upon the coverage, whether it be family, single, or single plus dependent. Date of eligibility for coverage begins on the 1st or 16th of the month, whichever comes after the date of hire.
Vision Insurance - Avesis
This is a stand-alone voluntary plan with premiums paid solely by the employee. The anniversary date of this plan begins July 1st of each year. Current employees who do not have coverage but wish to attain it may do so on the anniversary date. New employees have 30 days from date of hire to sign up for this coverage if they desire to do so.
